An affidavit is a sworn written statement of facts. In Pakistan it's used constantly — for name corrections, declarations of ownership, lost documents, marital status and more. Because it's sworn, getting both the wording and the formalities right matters. Here's how to do it.
What an affidavit must contain
- The heading 'AFFIDAVIT' and an optional subject line
- The deponent's full name, father's/husband's name, CNIC and address
- The sworn statements, written as clear points beginning with 'That…'
- A line confirming the contents are true and correct
- A verification clause with the place and date
- The deponent's signature
Stamp paper and attestation
For official use, an affidavit in Pakistan is normally printed on judicial stamp paper of the required value and then attested by an oath commissioner or notary public, who verifies your identity and that you have sworn to the contents. The stamp value depends on the purpose and the province, so confirm the current requirement locally before printing.
Example: name-change affidavit
"That my name has been recorded as 'Bilal M.' in some of my documents, whereas my correct and complete name is 'Muhammad Bilal'. That both names refer to one and the same person, i.e. me. That this affidavit is made to declare the correct name for all official purposes."
Common mistakes to avoid
- Vague statements — each fact should be specific and unambiguous
- Including anything you don't know to be true (a false affidavit is an offence)
- Forgetting the verification clause with place and date
- Using the wrong stamp paper value or skipping attestation
