The standard format
- Company letterhead, reference number and date
- 'TO WHOM IT MAY CONCERN' heading
- Employee name, ID, designation and department
- Dates of employment and the last working day
- A statement that the employee is relieved of all duties effective that date
- Confirmation that dues and clearance formalities are complete
- An optional line on satisfactory conduct, and good wishes
- Authorised signatory's name, designation, signature and company stamp
